Aetna members considered to be at high or moderate risk for hospital readmission within 30 days of discharge will receive support from a pharmacist in the form of in-home medication reviews or telephone consultations.

Why not use the pharmacist or pharmacy when the patient got thier meds........CVS is not going to have the complete list of meds for those patients and no zero history on the patient either....this will show nothing other than you paid out some money.
